Phyllis Hafichuk passed away November 3, 2009. She is predeceased by her parents Constance and Stephen Mellor, infant brother Reggie, brother Frank, sister Florence and son Ron. Phyllis is survived by her sisters: Doris Shabatka and Shirley (Morris) Stevenson; also by her children: Barb (Grant) Hannan, Barry (Hazel) Hack, Larry Hack, Sherry (Lloyd) Walters, and Leonard Hack, and daughter-in-law Karen Hack; thirteen grandchildren and twelve great-grandchildren. At Mom’s request, a private family service will be held at a later date. The family requests no flowers. Memorials to the Sask. Heart & Stroke Foundation, 279 3rd Ave., N., Saskatoon, S7K 2H8, would be appreciated. Her children give many thanks to the staff at Capilano Court, First Floor of the Moose Jaw Union Hospital, and Extendicare. In living memory of Phyllis, a tree will be planted in Besant Park by W. J.
