Passed away suddenly following a tragic accident on Saturday, May 3, 2008 at age of 46 years. Leaving to mourn his loving wife and best friend Lisa Chafe and two beautiful children, Daddy's little girl Maddie and Daddy's best boy Alex, his mother and father Dean and Gerard. Brothers Gary (Anna), Randy, David. Sisters Bonnie (Gary), Tracey (Mike). Parents-in-law Millie and Danny Chafe, sisters-in-law Lynn Dominic Arlene Luke, brother-in-law Daniel (Jason), also special niece Anna and nephew Mark, as well as a very large circle of family and friends and other nieces and nephews.
